This program enables organizations to identify patients with preventive care needs and provides reminders for preventive services based on USPSTF guidelines. This program enables care providers to intensify and streamline care coordination management during the critical transition from inpatient to home and over the course of the golden 30 day window post discharge. This is done using electronic monitoring of the patient’s condition which can trigger Telehealth interventions. It enables staff to provide non-face-to-face care coordination, such as communication on aspects of care, patient and/or family caretaker education to support self-management, independent living and activities of daily living, along with support for treatment regimen adherence and medication management with a documented audit trail for billing purposes.

Readmission Reduction Support

This program enables the intensified management of high risk patients using Pre and Post Discharge Assessments of their status, to avoid unnecessary readmissions and DRG holdbacks.

This program provides clinicians and care coordinators with patient reported data to more effectively manage episodes of care under Post Acute Bundled Payment Model 2 - Retrospective Acute Care Hospital Stay plus Post Acute Care and Model 3 – Retrospective Post-Acute Care triggered by Acute Care Hospital Stay and beginning at initiation of post-acute care services with SNF, inpatient rehab, LTAC or Home Health Agency. Pain Management Support

This program enables daily monitoring of patient pain and suffering to include pain level, location, quality, frequency and duration, medication adherence, behavioral/emotional issues and activities of daily living (ADLs).
