Does my electronic record already do these things?
No. Your electronic record is designed for providers to enter and exchange people’s medical information. It doesn’t allow people to independently communicate their needs, problems and preferences on their timetable. Things that are important to them, their family and their health care team to know, but for some reason, there’s no expedient means that empowers them to communicate their information and have their issues resolved. In some cases, an electronic record company may offer a personal health record (PHR) and, in other cases, providers may offer electronic portals. While most portals and PHRs offer the ability for patient’s to schedule appointments, request prescription refills and view an EMR data subset, neither enable a patient to self-monitor and communicate barriers to care and trace their resolution. Wellby does, while also giving providers the real-time data to understand how to best manage resources to ensure patients are having the best possible experience. What is Patient Engagement?
Health care should give each individual patient and family an active role in their care. Care should adapt readily to individual and family circumstances, as well as differing cultures, languages, disabilities, health literacy levels and social backgrounds.
